SCHOOL DENTAL EXAMS
The Illinois Senate recently passed a law that requires all children in kindergarten second and sixth grades in any public private or parochial school to have a dental examination and form completed by a licensed dentist on file in the school by May 15 of this school year.
The proof of dental examination and the waiver forms will be
uniform for statewide use.
The authorized form is attached to this newsletter. Many of our sixth graders
have already completed their examinations and returned a form for our school
file. For those of you who have yet to do so, you have until May 15 to return
the completed form. Your child’s report card may be held if the report
is returned by the end of the school year.
If you are not sure if you have returned a completed form, please call the school office. We have a report of all the sixth grade dental records. Thank you for your cooperation with this new state mandate.
