The following requests are guidelines when filling out your form (to simplify order processing) and picking up your order:

• You MUST INCLUDE YOUR PHONE NUMBER as this is your customer ID #.

• USE BLACK INK: no pencils, no gel pens, no red ink, and no return address labels. The scanner can not read these.

• Indicate QUANTITY WITH A NUMBER, not a check mark.

• Do NOT USE WHITE OUT. It melts onto the scanner. To 'erase', cross out the error and fill in the correct info above, below, or next to the line.

• To ensure the children's safety, USE OF THE BUS CIRCLE is prohibited until AFTER 3:35 OR when all buses have left the school grounds. Carry out assistance will be available.

• Please KEEP YOUR CHILDREN WITH YOU at all times during pick up. P.E. equipment and the boxes are off limits. This is for everyone's safety.

• To save on postage, forms will only be mailed to families without children at WES. If you leave town for more than one month and are on the mailing list, PLEASE notify me and I won't send forms during that time.
