AMY TODD FLEMING SCHOLARSHIP APPLICATION

Eligibility Requirements:

1. Must be a current senior at or graduate of Indian Creek High School.

2. Must have attended Indian Creek High School for one full year to be considered.

3. Must plan to attend a community college or university.

4. Must be attending that institution on a full-time basis for the entire year.

5. Must be a person whose intention is to enter the education field. Preferences will be given to the student who is pursuing a degree in special education.

Selection Criteria:

Selection will be based on character, personal merit, and the background of the individual. Students should be good citizens of the school and community. Applicants should exhibit the motivation to succeed and accomplish their personal goals. The selection process will be sensitive to the economic background of the applicant.

Extra-curricular activities demonstrating initiative will be evaluated, especially for leadership skills and cooperative spirit they represent. If, however, the student was required to work for economic reasons, this will be considered in lieu of extra-curricular activities.

There is no grade point average necessary for consideration. Rather we hope that this scholarship can be used by individuals who wish to further their education and are seeking ways to accomplish that goal.

Application and Selection Process:

Completed applications should be submitted to the Indian Creek Education Foundation Board at P. O. Box 426, Shabbona, IL 60550. These will be reviewed by a committee appointed by the Indian Creek Education Foundation Board. Awards will be made entirely at the Committee’s discretion. All applications will be remain confidential.

COMPLETED APPLICATION FORMS MUST BE RECEIVED BY THE BOARD BY Friday, March 31, 2008 IN ORDER TO BE CONSIDERED.
